If I am correct, what might be the problem is the page need to be refresh or the browser updated without you knowing and the pictures does not load fully. I had the same problem after my web browser updated and I couldn't log-in or the page showed the same thing.
Chrome does it only with it's current upgraded version.

looks like newer users only. Did you change any settings related to the storage location for avatars?

after a deeper look I found..

The difference between the working and non-working Avatars has to do with Permissions.
All of the non-working Avatars come back as "Forbidden, You do not have permission to access this document."

All of the images are there, but the access permissions are not allowing the images to show in the browser.
