Is it that bad? Especially for a young couple? Is there an optimal physical size? Didn't we use midwives for centuries on end?
There are probably a lot of optimal attributes that correlate with better chances. It's been years since I looked into it, but the impression I got was that giving birth is no joke. Infant mortality and maternal death rates among developing and developed countries was enough to convince me that medical care is important during child birth.
The unfortunate truth of the matter is there are simply a lot of mothers and children who don't survive without medical care. Every culture and ethnic group probably had numerous ways to try to deal with it, but scientific medicine provided game changing progress. Access to that care, seems to be the biggest hurdle to me since getting people to curb their birth rates is pretty difficult to do in a sane way.
