In the case of BP, full responsibility was shouldered by the firm, and its probably safe to say those responsible for the breach were held accountable, in some form or other; as representatives of the same team. I get that your second point, but I don't get why it's okay to treat BP as an entity like that when assigning blame for something negative but when talking about positive contributions it's not.
